What exactly do you expect as an answer?

Did Itch tell you to go to Paypal and issue a refund on the Paypal site?

Hi! No, the developers told itch.io to issue the refund but from the moment they did, itch.io stopped communicating altogether and did not issue anything despite my many messages. I'd like to know if it's okay for them and that my account won't get penalised. The whole thing has started more than a month ago...

If you read the message boards, you would know that Itch support is swamped. It just takes time.

If you already had an ok for a refund, you should have waited. Refunds on Itch are a manual thing. There is no automatic buyers remorse or anything like that. Buying digital goods that have no drm do not go well with such "return" policies. There need to be actual reasons, but from what I read from the message boards, they usually approve refunds. https://itch.io/docs/legal/terms#9-refunds

Opening a refund request over Paypal might cause trouble, depending on what you did exactly. If you opened an issue that the seller had to approve, that sounds fine. If you opened a dispute and forced a refund that might be not so good, especially if you made false claims in the dispute.

If you worry about consquences for not following the intended refund proceedings, the most likely consequences would be Paypal limiting your ability to open future disputes and Itch might be less inclined to approve any future refund requests. And neither will tell you so, so asking that very question is pointless and will probably result in no answer at all. But I have serious doubts that they would do so, after one incident.

For comparison, even Steam will limit your ability to request refunds, if you overdo this.

Forcing a refund is just not a good idea, if there are other options. There are big fees that might be applied in some scenarios. On Itch often a lot bigger than the actual payment price of the disputed digital item. A chargeback fee can be like $ 20. So, if you caused a chargeback that's financially very bad for the seller. If it was handled as a regular refund, I see no issues.

I did not force anything, I opened a dispute but it was accepted automatically from PayPal as soon as I opened it.

The refund was motivated by an issue with accessibility, so it was an actual reason that made the use of the game not possible for me.

There's a difference between not acting on it and not answering altogether or giving an estimated time of action. This was radio silence. How long do you usually wait to get a refund? If it takes more than a month, that's a problem... 

I don't think that it is pointless to try and talk with the platform about it though.
